LAW ENFORCEMENT OFFICER SAFETY ACT APPLICATION

The following tables list the items required to apply for a LEOSA credential. There are currently three different types of applications that can be filed, each for a different category of candidate.

Please select the application you will be filing and read each item carefully to make sure you have all the appropriate documents and information.

Notarized LEOSA Qualified Law Enforcement Application

To apply for a 926B Actively Serving LEOSA credential, a fully completed, signed and notarized copy of the LEOSA Qualified Law Enforcement Application is required. This application can be downloaded HERE.

After completing the application, it should be mailed in accordance with the application instructions.

Passport Photograph

Passport photograph as defined by the DoS for all U.S. passports (Photos in uniform are unauthorized).

Your photos or digital images must be:

  • In color
  • Sized such that the head is between 1 inch and 1 3/8 inches (22 mm and 35 mm) or 50% and 69% of the image's total height from the bottom of the chin to the top of the head. View the Photo Composition Template for more size requirement details.
  • Taken within the last 6 months to reflect your current appearance
  • Taken in front of a plain white or off-white background
  • Taken in full-face view directly facing the camera
  • With a neutral facial expression and both eyes open
  • Taken in clothing that you normally wear on a daily basis
  • Uniforms, clothing that looks like a uniform, and camouflage attire should not be worn in photos except in the case of religious attire that is worn daily.
  • You may only wear a hat or head covering if you wear it daily for religious purposes. Your full face must be visible and your head covering cannot obscure your hairline or cast shadows on your face.
  • Headphones, wireless hands-free devices or similar items are not acceptable in your photo.
  • If you normally wear prescription glasses, a hearing device or similar articles, they may be worn for your photo. Glare on glasses is not acceptable in your photo.
  • Dark glasses or non-prescription glasses with tinted lenses are not acceptable unless you need them for medical reasons (a medical certificate may be required).

Your passport image must be submitted along with your LEOSA application package.

Drivers License or Government Issued Identification with Photograph

A legible copy of a state driver's license or government issued identification with photograph is required for all LEOSA applicants and must be included as part of your LEOSA Application package.

Commanders Endorsement Letter

A commander's endorsement memorandum must be included as part of your LEOSA application package validating the individual meets standards established by the USAF and DoD.

NOTE: If you have retrained into a new career field without apprehension authority, you do not need to provide a commanders endorsement letter.

To apply for a 926C Qualified Retired LEOSA credential, a fully completed, signed and notarized copy of the LEOSA Qualified Law Enforcement Application is required. This application can be downloaded HERE.

Fingerprints for NCIC Background Check

Comprehensive background checks are required for all eligible applicants, with particular focus on violations of the Lautenberg Amendment to the Gun Control Act of 1968. Information relating to this process can be found on the application.

Do not submit your fingerprints as a part of your LEOSA package.

DISCLOSURE: Fingerprints are mandatory for all personnel who are requesting the Law Enforcement Officer Safety Act Credential. Failure to provide the information will result in a denial of your application.

Drivers License or Government Issued Identification with Photograph

A legible copy of a state driver's license or government issued identification with photograph is required for all LEOSA applicants and mailed in as part of your LEOSA application package.

Medical Declaration and Evaluation

MEDICAL EVALUATION MEMORANDUM IS ONLY REQUIRED FOR APPLICANTS WHO WERE MEDICALLY RETIRED

The original signed Medical Evaluation memorandum must be included in your LEOSA application package.

The required form can be download from HERE;

NOTE: The Medical Evaluation Memorandum MUST be signed by a DoD affiliated physician.

DO NOT modify the memorandum or send documents which may be categorized as Health Insurance Portability and Accountability Act (HIPPA) of 1996 information.

DD FORM 214 (Member 4 copy) / NGB Form 22

DD Form 214/Certificate of Release or Discharge From Active Duty (member copy 4 preferably) or NGB Form 22/National Guard Report of Separation and Record of Service or any other supporting documentation (i.e. official orders, performance reports, or decorations etc.).

The required certificate or documentation must be included in your LEOSA application package.

Note: These documents must show that upon retirement or separation the member had statutory powers of arrest or apprehension under section 807(b) of Title 10, United States Code (Article 7(b), of the Uniform Code of Military Justice).
